ABSTRACT
In this paper we present an analysis of the influence of public expense on healthcare in Brazilian cities and states in the period between 2000 and 2009 on the variation of the Human Development Index (HDI) -- Life Expectancy between the measurements of 2000 and 2010. The study comprises 5504 Brazilian municipalities based on data recorded on Institute of Applied Economic Research (IPEA). We established an indication of the effectiveness of healthcare expense and, as a secondary result, the relative efficiency of municipalities in healthcare expense was calculated. To this end, we used multiple regression with robust estimation. This technique correspond to the reproduction of the study of Barrios and Schaechter (2008), who used similar procedures to estimate the curve of effectiveness of public spending on education for the countries of the European Union (EU). It was found, in this study, a direct relationship between the municipal public health expense and variation of the HDI -- Life Expectancy index, revealing the degree of effectiveness of the expense. In addition, we obtained a relative assessment of the efficiency of health spending; that could be used in future studies when the causes of high or low efficiency found in the municipalities are identified. The main contributions of this paper are to identify the effectiveness of municipal public spending on healthcare, the adaptation of a model usually applied to nations for use in municipalities and a tool for calculating the relative efficiency of public expediture applicable in policy formulation in the area of healthcare.
